	We have 3 options for payments to your health & dental plans.
	Option 1: Send a cheque made payable to "Pacific First" to our mailing address:
		208 - 3970 Hastings Street, Burnaby BC, V5C 6C1
	Option 2: Add "Pacific First" to your payee list in your corporate online banking account using your plan number as the account number
	Option 3: Pay by credit card filling out the information below...
